LIVE IV Student Waiver Form

A free liability waiver form to help assure that students understand under what conditions they may practice IV’s on each other.

𝗘𝗠𝗦 𝗘𝗱𝘂𝗰𝗮𝘁𝗼𝗿𝘀: do you have students practice IVs on each other?

Ever considered the liability on that? One medical director even feared that allowing live sticks would encourage students to try OTHER ALS interventions, “Next, they’ll all be giving each other nitroglycerin!”

This form addresses BOTH issues. It provides a starting point and should NOT be construed as legal advice from a mere paramedic.

It gives the student the ability to acknowledge and accept risk or opt not to participate at all.

In addition, it reiterates the exact conditions under which practice is allowed. Therefore, wait and have students fill it out on the FIRST DAY YOU DO LIVE STICKS. This ensures the message is not lost in the firehose of the first week of paramedic school.

Take it, edit it for your state, pass it through legal at your organization. This is part of the 𝗘𝗠𝗦 𝗣𝗗 𝗥𝗲𝘀𝗶𝗹𝗶𝗲𝗻𝗰𝗲 𝗣𝗿𝗼𝗷𝗲𝗰𝘁—SHARING. None of us should EVER have to generate our own forms again!
